The DGK guideline "Sports cardiology and physical training for patients with cardiovascular disease" from 2020 states:
In view of the many benefits for the cardiovascular system, physical exercise should be encouraged in as many patients with cardiovascular risk factors or cardiovascular disease as possible. However, it should be borne in mind that exercise and sport can also trigger clinical complications [...] in these individuals. The following factors should be evaluated and taken into account when putting together an individualised training concept: Symptomatology, underlying cardiovascular disease and its impact at rest and during exercise, comorbidities and risk markers for future adverse events.

The Outpatient Clinic for Sports Cardiology is organised with this in mind. A dedicated assessment of cardiovascular risk factors and physical exertion is carried out in this consultation. Spiroergometry diagnostics can be used to determine the training zones and thus provide specific training counselling. The counselling should be available to patients with cardiac disease who wish to return to sport or take up a sporting activity.
