Your attending physician has decided that you will be discharged from the university hospital. This means that you will either be discharged directly home or require follow-up treatment as an outpatient or inpatient at another clinic or rehabilitation centre.
We have compiled further information for both situations below.
Ask for everything that is important to you in good time before you are discharged.
Here is a checklist
- You will receive a brief report for your family doctor.
- Detailed information on all examination and treatment results will be sent directly to your doctor.
- Do you need a certificate of incapacity for work or an insurance confirmation?
- Are follow-up examinations necessary?
- Which medication should I take and when?
- What should I pay attention to in terms of diet?
- Do you need a special diet?
- How can the outpatient nursing service be contacted?
Social facilities are important
- If you need to be injected with medication
- Wound dressings need to be changed
- You are not yet able to care for yourself
- The nursing staff will give you written instructions for the outpatient nursing service
- On request, you will also receive a leaflet with the most important social facilities outside the University Hospital and, if necessary, the addresses of self-help groups.
Please do not forget
- Take your personal belongings and deposited valuables with you and switch off your telephone
- Return borrowed books or aids in good time

Have you been discharged from hospital but still need follow-up treatment?
In order to ensure that your transition from inpatient treatment to appropriate follow-up care is smooth and optimally tailored to your needs, Ulm University Hospital carries out standardised discharge management.
After discharge from hospital treatment, further support is required in certain cases to ensure the success of your treatment. Appropriate follow-up care can include, for example, medical or nursing care, which can be provided on an outpatient basis or in inpatient rehabilitation or care facilities.
As part of this process, a multidisciplinary team will determine your post-discharge needs during your inpatient stay and initiate appropriate measures.
You will be informed about discharge management by our friendly admissions staff on admission and your consent will be obtained. Your consent enables Ulm University Hospital to organise the process to meet your needs and, if necessary, to involve your health or care insurance fund for support.
At the beginning of your stay at Ulm University Hospital, the dedicated nursing staff will assess your current care needs in your home environment in an admission interview (initial assessment). During the course of your treatment, your anticipated needs after discharge will be assessed (dedicated assessment) and any necessary measures initiated.
At the end of your treatment, our specialist doctors can prescribe a limited amount of medication, remedies, aids, sociotherapy and home nursing care to bridge the gap until the next working day from your GP or registered specialist and determine your incapacity for work.
If the assessment of your needs after discharge shows that you require follow-up measures immediately after your hospitalisation, such as accommodation in a rehabilitation facility or respite care, etc., our competent Social Counselling Service team will advise you in detail and assist you with the application process.
