Bridge care and specialisedoutpatientpalliativecare(SAPV) are aimed at patients with an incurable, advanced illness who require more care and want to spend as much time as possible at home.
- Bridge care is a special hospital service
for the palliative care of tumour patients. - SAPV is a specialised outpatient palliative care service
for all seriously ill and dying patients.
It is an additional service to the care previously provided by your family doctor and nursing service.
Both services are free of charge. Bridging care is financed by the hospital and SAPV is financed by the respective health insurance company after an assessment of the entitlement.
The service includes inpatient discharge management and further care at home.

Clarification of the current situation in consultation with the hospital doctors, carers and other people involved in the patient's care
Forward-looking planning and organisation for the home, hand in hand with the family doctor and other parties involved

We are your contact for home care, for questions of all kinds, for crisis situations, for instruction needs and much more

Care via SAPV is provided in co-operation with the Ulm Palliative Care Network (PNU).
