Institute outpatient clinic
Following the establishment of the University Clinic for Child and Adolescent Psychiatry / Psychotherapy in autumn 2001, the Institute Outpatient Clinic was set up as a contact point for the examination and treatment of children and adolescents with mental disorders. Doctors, psychologists, child and adolescent psychotherapists, educators, social pedagogues and medical assistants work together in the outpatient clinic to provide diagnosis and treatment.
Children and adolescents from 0-18 years of age can be seen, as well as adolescents up to 21 years of age with prior authorisation from the health insurance company. Treatment in the outpatient clinic is intended to avoid or shorten costly and often drastic inpatient treatment for families. The treatment is paid for by the health insurance company. As a rule, referrals are made by GPs, paediatricians or specialists in child and adolescent psychiatry and psychotherapy. Direct registration by parents/guardians or adolescents is possible. In an emergency, an appointment can also be made without an appointment.
According to the German Social Security Code V §118 (1), the services of the child and adolescent psychiatric outpatient clinic are aimed exclusively at mentally ill children and adolescents who, due to the nature, severity and duration of their illness, are dependent on treatment in a hospital outpatient clinic.
We therefore ask for your understanding if we are unable to offer treatment to patients who can be adequately treated by child and adolescent psychiatrists or child and adolescent psychotherapists in private practice and refer them to the services offered by those in private practice.
Diagnostics and therapy
In an initial consultation, the child's problems are analysed against the background of the family, school and social environment. An initial clinical examination and assessment is carried out. Further diagnostics then include a psychological test, a physical-neurological examination and possibly further medical examinations as well as in-depth explorations with the patient and caregivers, as required. The results of the examination are then used to make a diagnostic classification and a treatment plan is drawn up in consultation with the patient and carers.
Our treatment options include counselling, psychotherapy, family therapy, drug therapy, group therapy, parent training and crisis interventions. In many cases, an important part of the help is the activation of resources and social support measures. If necessary, we coordinate community-based help and establish contact with authorities, schools, counselling centres and other institutions. The aim of treatment is to restore the best possible quality of life for patients and their families, even in the case of chronic disorders.

All paediatric and adolescent psychiatric disorders are treated. There are also special consultation hours for individual disorders.
According to the German Social Security Code V §118 (1), the services of the child and adolescent psychiatric outpatient clinic are aimed exclusively at mentally ill children and adolescents who, due to the nature, severity and duration of their illness, are dependent on treatment in a hospital outpatient clinic. We therefore ask for your understanding if we are unable to offer treatment to patients who can be adequately treated by child and adolescent psychiatrists or child and adolescent psychotherapists in private practice and refer them to the services offered by those in private practice.
We first carry out a detailed psychiatric, medical and psychological test diagnosis. The findings are then discussed with the patient and their carers before we recommend treatment. Depending on the existing disorder and the problems described, it is possible to take advantage of various disorder-specific individual and group therapy services and parent counselling. Drug treatment is also possible in our outpatient clinic, as is referral to inpatient treatment if necessary. Another important component is the exchange with schools, youth and social services, counselling centres and other institutions, if desired. We are also happy to assist with referral to outpatient psychotherapeutic or functional treatment.
We are happy if it is possible for us to meet both biological parents during the initial consultation. Please note that the consent of all legal guardians is required for treatment.

Who is the offer aimed at?
The fact that children and adolescents experience difficult times and situations as they grow up is part of normal development.
Behavioural and emotional problems are therefore not always a sign of mental illness. However, if they persist, this is a reason to seek professional help.
The most common disorders that can be treated with behavioural therapy are

Children, young people and adolescents up to the age of 21 are treated in the outpatient clinic for behavioural therapy.
The prerequisite for successful treatment is the active co-operation of patients and relatives. Patients should be able to attend regular weekly treatment appointments. Furthermore, regular accompanying discussions with the patient's legal guardians are always an integral part of psychotherapy with children.
First steps
Once you have registered by telephone, you will first be given an orientation appointment at our consultation centre. This is followed by several diagnostic appointments, which serve to verify the existence of an illness requiring treatment and the subsequent treatment planning.
In the run-up to possible psychotherapy, a medical consultation is also obtained to rule out a physical cause of the disorder.
After these initial steps, a decision is made as to whether an application should be made to the health insurance company to cover the costs.
If the application is approved, psychotherapy can begin. As a rule, one 50-minute session is then held each week.
The duration of treatment depends on the type and severity of the mental illness. On average, a treatment period of 6 to 12 months can be expected.
Costs
Costs for consultation appointments and preparatory diagnostic appointments are generally covered by the statutory health insurance funds. If there is an illness requiring treatment, the costs of the subsequent psychotherapy are also covered by the statutory health insurance funds. Patients are not required to make a co-payment.
The AZVT
The psychotherapy outpatient clinic is part of the Training Centre for Behavioural Therapy (AZVT) at the Ulm Clinic for Child and Adolescent Psychiatry/Psychotherapy. This has been in existence since 2009 and is state-recognised as a training centre for child and adolescent psychotherapists.
A central objective of the AZVT is to ensure a particularly high quality of treatment in the outpatient clinic.
This is guaranteed by several factors:
The training therapists have a degree in psychology, education or medicine and are at an advanced stage of their psychotherapy training. They are guided by high standards and have access to the necessary up-to-date diagnostic procedures and treatment manuals in the outpatient clinic. They are also supported in the implementation of therapies through continuous guidance and supervision as well as "clinical back-up".
Close cooperation with the other clinical departments at the University Hospital ensures that the latest treatment guidelines are followed, based on the latest scientific findings from psychotherapy research. This means that the most suitable form of treatment for the patient at any given time can be selected.

With a frequency of >1% of the population, autism spectrum disorders are anything but rare causes of developmental and behavioural problems.
Many parents are concerned at a very early stage in their child's development about their child's sluggish or stagnant speech development or pronounced sleep and feeding problems. In some children, unusual-looking movement peculiarities such as waving their hands or twisting their fingers can be observed (so-called stereotypies). However, a lack of eye contact, a lack of interest in social interaction and extreme irritation at any changes are also symptoms that are frequently reported. The special perception and perception processing in the brain of those affected also often lead to social problems and can result in psychological and behavioural problems such as withdrawal, depression, but also aggressive/auto-aggressive behaviour. A large number of children affected by autism are cognitively impaired. But even those who have good cognitive and language resources are so limited by their deficits in communication and interaction skills and the resulting effects that they are hindered in their participation and integration into society. They often have attention problems, impulsive behaviour and motor restlessness, epilepsy or anxiety disorders. People affected by autism therefore usually have multiple disabilities. Raising, caring for and living with autistic children presents families with autistic children with very special challenges.
In addition to a disorder-specific assessment in cases of suspected autism spectrum disorder, the consultation offers psychiatric treatment, social counselling and educational and educational services for caregivers. We also attach great importance to cooperation and dialogue with the care facilities and supporting institutions (e.g. kindergarten, school, youth welfare office).

Emotionally unstable personality disorders of the borderline type are characterised by severe problems with impulse and affect regulation, which can manifest themselves in rapidly changing moods, feelings of inner emptiness or explosive aggressive outbursts, as well as abnormalities in relationships, whereby relationships are often intense but unstable. This constellation of symptoms is one reason why emotionally unstable personality disorders are not a rare clinical diagnosis even in adolescence. Non-suicidal self-harming behaviour (NSSB) and suicidal tendencies in particular are symptoms of BPD, but can also occur independently of the diagnosis and have a significant impact on the development of adolescents. For the treatment of BPD in adolescence, the techniques established for adulthood have been adapted to the developmental level and needs of adolescents. Dialectical behavioural therapy for adolescents (DBT-A) should be mentioned here, for which there is already evidence of effectiveness. The outpatient treatment programme according to DBT-A generally includes participation in individual therapy and weekly skills training in a group ("skills group"). Before therapy begins, a detailed diagnosis, treatment planning and examination of the motivation for therapy are carried out.

Our early childhood consultation hour offers parents with children from birth to the age of three support and diagnostic clarification for children with regulatory disorders. Child regulation disorders include, for example, sleep disorders, feeding disorders, excessive crying in infancy as well as behavioural problems and developmental disorders in infancy.
The aim of the consultation is to recognise risks in the parent-child relationship and child development as early as possible and to strengthen parental skills. By recognising and treating behavioural problems in early childhood, the aim is to counteract possible developmental delays in good time.
As part of an initial presentation at our clinic, the "problem behaviour" is first recorded in detail. If necessary, and with the parents' consent, an external medical history is obtained from other key caregivers such as the family midwife, nursery teacher, grandparents, etc. In addition, developmental neurological and developmental psychological examinations may be carried out. The results are discussed in detail with the parents. Following a counselling process, various therapeutic approaches are available depending on the wishes and requirements:
- The implementation of developmental psychological counselling (video-supported parent-child counselling concept)

The children's home consultation offers child and adolescent psychiatric diagnosis, treatment and counselling for children and adolescents living in youth welfare facilities. Some of the child and adolescent psychiatric consultations take place in outreach form in the respective facilities. The aim of the children's home consultation is to identify behavioural problems and the need for treatment in children and adolescents in institutional care as early as possible.

Children and young people who live in youth welfare centres suffer from mental disorders at an above-average rate. These children have often already had traumatic experiences in their families or behavioural problems or developmental disorders have made placement in a youth welfare facility necessary. Especially when children and adolescents are ambivalent about being placed, it can also be difficult to build relationships with staff, and children and adolescents may fear losing contact with their parents. Even biological parents who have placed their child in the care of a youth welfare centre often express the fear of no longer seeing their child regularly or losing their special role as parents.
It is therefore important for the healthy development of children and adolescents to have a clear and reliable care framework, cooperation between biological parents, the facility, the youth welfare office and other institutions, as well as consideration of individual psychiatric and psychotherapeutic treatment needs.

Some children and adolescents suffer from strong emotions that are evoked in them when they hear certain sounds such as smacking and swallowing noises. These emotions can be so intense that they actively avoid the situations in which the sounds occur. This considerably restricts their ability to function. This phenomenon is called "misophonia" in research. Misophonia literally means "hatred of sounds". People with misophonia experience feelings of anger and disgust when they hear certain sounds that are normally produced by people. These are often mouth and throat noises such as smacking, swallowing and breathing. However, it can also be the sound of repetitive movements (e.g. shuffling feet).
We offer diagnostics and counselling for misophonia as part of our special consultation hours. During the diagnosis, the various misophonic triggers (= misophonic noises), the severity of the emotional reaction when confronted with the specific triggers and various accompanying illnesses that can be associated with misophonia are recorded. The subsequent counselling includes information on the disorder, the creation of an individual explanatory model for the symptoms and information on possible forms of treatment, which are closely based on the current state of research.
The treatment and diagnostic services are aimed at children and adolescents with behavioural problems or mental disorders in connection with a genetic syndrome, a mental or physical disability or a somatic illness.

Children and adolescents with genetic syndromes, mental or physical disabilities often show behavioural problems in addition to varying degrees of developmental disorders, which cannot be conclusively clarified and treated in the context of paediatric or neuropaediatric care, especially as the accompanying physical health impairments are often in the foreground. Children and adolescents with intellectual disabilities in particular have a significantly increased risk of mental disorders.
However, the consultation is also aimed at children and adolescents who suffer from psychological symptoms and/or behavioural problems in connection with a serious or chronic physical illness. In cooperation with the Rheumatology Department of the Clinic for Paediatrics and Adolescent Medicine, we also offer an assessment in cases of suspected Long COVID.
Due to the complexity of the diseases, transparent and close cooperation with all treating doctors from other specialities is a prerequisite for a presentation in our special consultation. This also includes the presentation of preliminary findings upon registration in order to avoid stressful duplicate diagnostics and to ensure optimal planning. We ask for your understanding that we can only act as a second opinion centre for patients who live outside Ulm, Neu-Ulm or the Alb-Donau district. In these cases, the registration must be made with a corresponding question by the treating colleagues on site.

Foster children are usually children with stressful previous experiences, which can make it difficult to build a relationship with the foster parents. Mental disorders and developmental delays are common in foster children. Foster parents are often confronted with behavioural problems and challenges in dealing with the child entrusted to them, which they feel unable to cope with without support. Caring for the foster child expands the foster family's network of relationships. At the same time, well-coordinated cooperation with biological parents, the youth welfare office and other institutions (school, day care centre, early educational support, therapists, etc.) is necessary; without support, it can be difficult to reach agreements that are satisfactory for all parties involved. For various reasons, biological parents find themselves in the situation of having to place their child in someone else's care. Sometimes they fear not being able to see their child often enough and losing their special role as parents or their relationship with their child, they feel guilty or have mixed feelings towards the institutions and social services. Foster parents are often faced with the challenge of building a relationship with a child whose past they only know to a limited extent, while at the same time supporting contact with the biological parents and managing transitions between the foster household and the parental home. Foster children, on the other hand, are caught between different caregivers and institutions that look after them and can feel pulled back and forth if the adults fail to cooperate. They often have to learn to deal with difficult feelings such as anger, feeling rejected, guilt or worry about their parents and siblings. The establishment of a clear and reliable care framework, cooperation between biological parents, foster parents, youth welfare office and other institutions, as well as the consideration of individual psychiatric-psychotherapeutic treatment needs are important for the children's well-being and the restoration of their mental health.

Many children and young people ask themselves about their gender identity and belonging. Some of these children and young people have a clear inner conviction regarding their gender identification as a "boy" or "girl" and experience this as "right" for them. Other children and adolescents react with strong insecurity regarding their own gender identity and not least with anxiety or depression. This uncertainty is often also reflected in the family environment. Parents in particular are faced with major challenges when it comes to questions about their child's gender identity and sexual orientation. As part of our special consultation hours, we offer open-ended counselling and examination of children and adolescents with the involvement of their parents and caregivers in order to clarify the possible existence of gender incongruence. The diagnosis and exclusion of other psychological problems or organic diseases includes an interdisciplinary exchange with medical colleagues, in particular from the Department of Paediatric Endocrinology. The subsequent counselling and, if necessary, therapeutic support is closely geared to the needs of the children and adolescents and their parents and to the existing symptoms. It ranges from simple counselling with regard to gender identity in the case of existing sexual maturation crises to therapeutic support in line with guidelines in the case of a desire to adapt to the opposite sex after careful examination of all available criteria.

Children and adolescents who have been exposed to extremely stressful or threatening (so-called traumatic) events may subsequently develop symptoms of psychological stress such as sleep disorders or anxiety or have difficulties coping with everyday life. These are completely normal and typical reactions to stressful life events, which in most cases recede after a few days or weeks. It is therefore not initially necessary to start psychotherapeutic treatment immediately after a stressful event and there is no benefit in terms of the long-term prognosis. Our acute consultation hours therefore initially focus on counselling and monitoring the course of the symptoms. If the symptoms persist for several weeks, further diagnostics can be carried out and disorder-specific psychotherapy (trauma therapy) can be initiated.
